If you'd like to transfer to our practice, please note that we cannot make an appointment for you unless your child's previous records have been signed off on by Dr. Kushwaha. Should your previous practice need a form from our letter head please download the Release of Records form from the link below.
Once an appointment has been made, please print out the New Patient Registration Packet. You may download the packet below to print and complete at your convenience. Please bring these forms to your appointment. Please fill out one set per child, including the Release of Records form. If you choose to fill out paperwork in office, please plan to come at least 15-20 minutes prior to your appointment time.
